PHYSICALS
Any student wishing to participate in athletics during the school year must:
1. Have this physical form completed by a doctor after April 1st of 2023 (valid until May 31, 2024), and
2. Read/Agree to Code of Conduct on Final Forms. 

All athletes must complete the steps above and upload ONLY THE SIGNED DOCTOR'S PAGE (page #3) of the physical with special attention to the pink highlighted areas at the bottom of the page.  No hard copies of physicals will be accepted.
